Option 2 — Phone, Fax or Postal Mail.
Note: if you order via phone, fax or postal mail, you will not qualify for any online monthly specials and/or discount or special offers that are valid only with online orders!
Step 1: Download the Consumer Order Form. (Make sure you have Adobe Acrobat Reader installed before you download the order form.)

Mac users hold down the mouse button over the link to save the file onto your computer.
